Description
Clinical Liaison (RN) - Acute Care at Home
Location: Washington State (On-Site / Embedded at Partner Medical Facility) Employment Type: Full-Time Compensation: $100,000 - $105,000 / year + Quarterly Bonuses
The Role
Are you a Registered Nurse (RN) looking to step away from the bedside and transition into an administrative and business development role? We are seeking a dynamic, mission-driven Clinical Liaison to serve as the primary bridge between our hospital partners and the myLaurel care team. In this fully on-site role, you will be embedded directly inside our partner medical facility. You will serve as the face of our Acute Care at Home program on the hospital floor, focusing on relationship-building, business development, and administrative coordination to drive referrals and ensure seamless patient handoffs.
(Note: While highly ideal for an RN transitioning to business development, experienced Paramedics and Medical Social Workers with a flair for relationship-building are also strongly encouraged to apply!)
What You’ll Do
- Drive Business Development: Act as the primary relationship manager on-site. Work alongside hospital case managers, discharge planners, emergency department staff, and physicians to generate referrals and grow our in-home care programs.
- Lead Administrative Coordination: Manage the logistical and administrative workflows of patient transitions, ensuring that the handoff from the hospital bed to the patient’s living room is flawless.
- Identify Eligible Patients: Use your clinical background to review patient charts on the floor, assess eligibility for our acute-care-at-home model, and advocate for safe alternative care pathways.
- Educate & Advocate: Conduct presentations and daily one-on-one touchpoints with facility staff to educate them on myLaurel’s capabilities, clinical criteria, and patient benefits.
- Solve Problems Strategically: Identify operational barriers to patient referrals in real-time and collaborate with the myLaurel leadership team to implement high-level administrative solutions.
What You Bring
- Active Clinical License: Current, unencumbered license in the State of Washington as a Registered Nurse (RN), Paramedic, or Licensed Clinical Social Worker (LCSW/LMSW).
- Experience: 2+ years of experience in clinical care. Previous experience or a strong interest in case management, discharge planning, business development, or clinical sales is highly preferred.
- Relationship Builder: Excellent communication and interpersonal skills, with the natural ability to establish trust, integrate into a facility's culture, and build rapport with busy medical professionals.
- Administrative Acumen: Strong organizational skills and a desire to manage program logistics rather than direct, hands-on patient care.
- On-Site Presence: A highly engaged professional who thrives in a bustling medical facility environment and is comfortable being the primary, in-person representative for myLaurel.
- Tech-Savvy: Comfortable navigating EMRs (Electronic Medical Records) and utilizing CRM software to track referral metrics and business growth.

About myLaurel
myLaurel was founded on the belief that a meaningful portion of hospital care can—and should—be delivered in the comfort of home. As a tech-enabled medical group, we provide on-demand acute and transitional care to frail, elderly, or medically complex patients at home—helping them avoid the traditional pathway of ambulance, emergency department, hospital admission, and post-acute facility care.
Health systems, payers, and provider groups partner with myLaurel to reduce avoidable utilization, lower readmission rates, accelerate throughput, and improve the patient experience—all while maintaining the highest standards of clinical quality and safety. We stand behind our outcomes by putting our fees at risk, demonstrating both accountability and confidence in the results we deliver.
Care is delivered through a physician-led model that combines virtual oversight with in-home clinicians, advanced technology, diagnostics, and therapies. For high-risk populations, myLaurel achieves 33% lower ED utilization, a 49% absolute reduction in readmissions, and an average of 15 bed-days saved daily - alongside a Net Promoter Score of 97.
