Job Summary The Patient Safety Officer manages one or more hospitals. They provide leadership and expertise to establish a culture of safety. This minimizes hazards and patient harm by applying the BSWH Safety and Quality Plan: Guiding values. The PSO examines and integrates data from internal and external resources. They work collaboratively to develop programs that support Zero Preventable Harm. *Specific Job Summary for Centennial* Minimal oversight therefore RN must be a self-starter and visble to physicians, co-workers and all parties involved with the shared patient. Will manage only one hospital with assignments as follows but not limited to: 0.5 Sepsis Coordinator * Concurrent audits * Open communication with providers and staff * Holding local sepsis meeting * Attending system meetings * Rounding on inpatient sepsis patients 0.5 Patient Safety Officer * Attend post-fall huddles * Manage Riskonnect entries/responses * Handle Leap Frog surveys, meetings * Rounding with physicians and staff Essential Functions of the Role * Assumes the leadership role for a safe culture by sharing accountability for PS throughout the facility/organization. * Oversees the management and use of event information to benchmark and track progress to zero preventable harm. * Works with facility managers, as well as clinical risk management to ensure thorough and timely event management. * When adverse events cause harm to patients, the PSO will participate in the implementation of an early intervention action plan. * Trains new facility managers on the event reporting system. Establishes processes and expectations for thorough, credible, and timely event reviews and management. * Communicates the Patient Safety strategy, tactics, and status updates regularly to hospital administrators and other key stakeholders. The PSO meets regularly with facility managers, clinical risk management, and managers. They discuss and develop next steps or actions for adverse events that meet established criteria. * Is a trusted agent for clinical staff to report concerns about aspects of care that may result in patient harm. * Collaborates with facility and system managers to develop, adopt, implement, evaluate, and sustain policies, procedures, guidelines, and toolkits. These processes support meeting publicly reported and internal measures, including but not limited to: * Hospital Acquired Infections / Hospital Acquired Conditions * Patient Safety Indicators (PSIs) * Falls Prevention Program & Reduction * Hospital Acquired Pressure Injuries * Medication Management (i.e. Infusion Pump Compliance, Bar Code Medication Administration) * TDSHS Preventable Adverse Event Reduction * Facility Patient Safety Dashboard/Scorecard * Serves as the facilitator for Leap Frog submission. * Review and ensures compliance with National Patient Safety Goals. * Serves as chairperson or member of the Hospital's Patient Safety (PS) Committee. * Assists with developing process improvement plans and policies. Performs evaluations, examines reports, and ensures compliance with quality, performance, and Patient Safety requirements. * Decreases future harm risk by evaluating and redesigning systems to improve care processes, like pressure functions, checklists, and human factors. * Manages patient safety initiatives, educates staff on patient safety, and promotes overall compliance and improvement. * Provides data from AE reports to support and monitor safety initiative programs for high-risk patient populations. * Participates in Patient Safety & Zero Harm Rounds to identify risks. Empowers staff to identify and resolve patient safety concerns. * Regularly reports PS data, and updates on PS programs/initiatives to facility executive leadership, managers, and medical staff administrators. * Represents the facility at various system committees or councils. Participates in process improvement initiatives within those committees. Supports the spread of best practices within their facility. Key Success Factors * Certified Professional; Patient Safety (CPPS), Healthcare Quality (CPHQ) or Healthcare Risk Management (CPHRM) highly preferred. * Experience in patient safety, quality, or risk management, highly preferred. * Effectively report, investigate, and evaluate patient safety incidents, medical errors, and potential risks using the Adverse Event (AE) reporting tool. This can be done by thorough chart review and/or interviews as needed. Adverse Event reports are reviewed daily within the standard work week or when leadership notifies of an event. Work collaboratively to develop responsive programs that support a culture of patient safety and reduce adverse events or hazards. * Provides data from the Event Reporting System to facility managers. This supports and monitors harm reduction programs' effectiveness, like fall reduction. Creates a trusted environment for employees to report concerns and maintains confidentiality. * Improve consistent delivery of evidence-based care and reduce preventable harm. Focus on reliability and use a structured framework to collect and examine data on outcomes and processes. The Model for Improvement (PDCA) is used in improvement processes at BSWH. * Supports and facilitates systems and processes to promote safety and reliability. This helps reduce preventable patient harm. * Partner with facility leadership to create activities that support a fair and consistent environment. This promotes learning, safe systems, and manages behavioral choices related to patient safety. Examples include Patient Safety Rounds and Event Response.
