Willow Brook Home is seeking a full-time MDS Coordinator to support our 50-bed care community, known for its steady leadership and collaborative clinical team.
In this role, you’ll take ownership of key clinical processes that require accuracy, consistency, and judgment:
Completing MDS and CAA assessments thoroughly and on schedule
Participating in and contributing to care plan conferences
Developing and refining individualized nursing care plans based on resident needs
Working closely with the interdisciplinary team to support quality outcomes
Stepping in as a staff nurse as needed to stay connected to resident care

Requirements
We are specifically looking for candidates who bring:
- Strong, hands-on experience with MDS and Medicare reimbursement (preferred and highly valued)
- Active RN license in Ohio
- Confidence in documentation, timelines, and regulatory expectations
- Clear communication and a detail-oriented mindset
