One of our clients is seeking Outreach Case manager in the Germantown area of Philadelphia. Position is 85% hybrid or out in the community. Must have lived experience with homelessness & dependency.

CLIENT'S AVAILABLE HOURS
9:00 am- 5:00 pm Monday - Friday

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Conduct comprehensive assessments of clients' strengths, needs, and goals within the first 30 days of enrollment
* Develop and implement individualized recovery support plans
* Facilitate support groups, workshops, or educational sessions as needed.
* Weekly schedule of case management and outreach sessions
* Other duties as assigned

CLIENT'S REQUIRED SKILLS & EXPERIENCE
* Bachelor Degree in criminal justice, psychology, social work, sociology, counseling.
* 1 year experience as case manager
*Lived experience with homelessness and dependency
* Ability to engage with adults dealing with addictions
* Some experience working in addictions field
