Horizon Public Health, serving Douglas, Grant, Pope, Stevens and Traverse Counties, is
recruiting a compassionate and skilled Registered Nurse to provide case management services
for clients under age 65.

Minimum qualifications include a Baccalaureate degree in Nursing and licensed in MN with at least one year of home and community-based experience. Exceptions may apply for ADN applicants with at least two years of home and community-based experience. Prior experience in long term care is also beneficial. Successful applicants must pass a criminal background check and a pre-employment drug test. Must have valid MN drivers license and reliable, insured transportation. This is a Grade 13 MNA/AFSCME Council #65 union position.
