Description
Position Summary:
The Adult Foster Care (AFC) Nurse provides clinical oversight, home-based assessment, and coordinated support services for individuals with medical needs and/or developmental disabilities within the AFC program. This role utilizes a collaborative, team-based approach to ensure high-quality, person-centered care and regulatory compliance. ***LPNs encouraged to apply as well***
Schedule: 24 hours/week
Compensation: $36 - $38/hour
Key Responsibilities:
Clinical Oversight & Service Delivery
- Participate in the evaluation of potential AFC members and prospective caregivers.
- Conduct clinical assessments and develop individualized, multidisciplinary care plans.
- Provide clinical supervision and ongoing support for assigned AFC placements, including monthly home visits and case management.
- Monitor participant health status on a continuous basis and respond to changes promptly.
- Provide education to members and caregivers regarding medical concerns, prevention strategies, and wellness practices.
- Coordinate community medical services and personal care supports for members as needed.
- Collaborate with primary care providers, community service providers, medical professionals, and internal team members to ensure integrated care.
Documentation & Compliance
- Complete and/or review all required AFC program documentation in accordance with agency standards.
- Maintain compliance with Kennedy-Donovan Center mission, philosophy, policies, procedures, and quality standards.
- Ensure adherence to all applicable state and federal funding agency certification requirements and regulations.
- Maintain certifications and competencies required by the agency and funding sources.
Collaboration & Communication
- Serve as a resource for caregivers, case managers, and interdisciplinary team members regarding clinical needs.
- Communicate effectively with primary care providers, caregivers, families, and program staff to share updates on participant health and support needs.
Professional Development & Additional Duties
- Participate in professional development activities to enhance clinical skills and maintain licensure.
- Perform other job-related responsibilities as assigned.
Requirements
Required Qualifications:
- Current Massachusetts Registered Nurse (RN) licensure.
- Minimum of 3 year of experience in human services, long-term supports, disability services, or a related program area.
- Understanding of home- and community-based service models, AFC, Shared Living, or related MassHealth programs.
- Excellent communication, organizational, and interpersonal skills.
- Proficiency with Electronic Health Records (EHR), and Microsoft Office Suite.
- Valid driver’s license and reliable transportation for travel.
- Ability to complete all required trainings required such as CPR and First Aide
Preferred Qualifications:
- Experience working in community-based programs.
- Experience with Medicaid, MassHealth, and state funding systems.
- Bilingual or multilingual skills.
