The Skilled Rehabilitation Facility (SNF) Senior Case Manager is responsible for leading clinical operations and medical management activities across the continuum of care. In close collaboration with Providers (MDs/APRNs), facility staff, and interdisciplinary teams, the role ensures high quality, coordinated, and patient centered care. This position oversees comprehensive case management activities including assessment, planning, implementation, coordination, monitoring, and evaluation of member care needs.
The Senior Case Manager also provides health education, coaching, and treatment decision support while serving as a key point of contact for transitions of care and inpatient coordination.
Primary Responsibilities
- Utilize approved clinical criteria to assess and determine appropriate levels of care for Skilled Rehab members
- Apply motivational interviewing techniques to enhance member engagement and support individualized goal setting
- Provide member education, conduct medication reconciliation, and support benefit management
- Ensure timely follow through with the Care Navigation Team, including appointments, referrals, and access to health plan and community resources
- Facilitate communication among patients, families, providers, and facility staff, advocating for members as needed
- Ensure coordinated delivery of services and appropriate utilization of available programs
- Communicate clinically relevant information with internal and external stakeholders to support seamless care coordination for inpatient members
- Serve as the identified point of contact for managing transitions of care, including discharge planning
- Support medical management consultation and continuity of care initiatives across care settings
- Ensure required Medicare compliance

Required Qualifications:
- Current, unrestricted Nevada RN license
- 3+ years of direct patient care nursing experience
- Confidence learning and utilizing multiple software systems
- Unrestricted Driver's License. Reliable transportation for travel to patient and client sites within the assigned area
- Solid clinical judgment and commitment to continued professional development
- Demonstrated ability to organize, prioritize, and manage complex caseloads
- Solid verbal and written communication skills
- Ability to work independently, adapt readily to change, and maintain flexibility in a dynamic work environment
Preferred Qualifications:
- Experience using healthcare practice management systems
- Utilization Management/Case Management experience
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $72,800 to $130,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
